MTOP & Franchise
Requirements
- Certificate of Registration with Official Receipt (CR/OR), Certification of Sale, Deed of Sale (If Necessary).
- Approved inspection report with sketch of operator residence at the back.
- Certification of FEMATODA.
- Barangay Clearance.
- CTC or Cedula.
- 2 x 2 picture (for driver ID).
- Franchise (previous).
